Stopped receiving Notification Email

    The second issue is with the Jot forms as well. We have 5 submission forms on our website, one being a contact us message form. With the four other forms, there are two that are having issues. As of 12/8/16, The Corporate & Orders of Four Plus and the Request a Brand form have stopped being updated on the Jot form website. In addition, we stopped receiving emails for any submitted forms through these two.


    Due to another web monitoring program that we use called Inseev, we have been able to verify these forms still work and are getting submissions which we get emails from through Inseev.

    Latest submission date of two forms that you mentioned is 2016-12-08. I have tested your embedded form on this page  and form can not be submitted. Submit button is stuck on "Please wait...". 

    After that I have tested you standalone form and form was successfully submitted. 

    You have embedded your form with full source code. Please note when form is embedded with full source code, that each time your form is modified you will need to re-embed your form with new code. You need to re-embed both of your forms. 

    I would suggest you to use iFrame code. Inside of this guide you can find how to get iFrame code of your form:

    unable to click on upload, changed to iframe, still nothing, unable to choose a file
