When we receive this form in our email inbox, the sender info is their email address and it used to be and is on our other forms the name assocaited with the email address. How do fix that??? Thank you!
Sender used to say Jacque Preble
now it just says email@example.com

Please provide us the FORM ID so that we can check.
Probably there an issue with your Notification settings. Kindly check the Sender Name dropdown if it's set to Full Name or whatever field you assign to gather names.
Go to Setup & Embed > Email Alerts > Notifications > Reply to and Recipient Settings
