1. the logic isn't sending to the correct pages
How can we help?
if you choose "shipping items" it should go to the "shipping items" page, it's not.
It's the same for all answers to "how can we help?"
2. Once you've gone to your dedicated page based on #1, each page should submit and go to the end. You can only submit one kind of request per form, so there should not be a "next" or "back"

It seems that the conditions set on the form are correct. It looks like the page labels are not displaying correct. Could you try removing the conditions and set them again by considering the page numbers? It is not required to setup a condition for Shipping items since the next button will automatically take the form to next page itself.
I'll investigate further on the page labels and get back to you with relevant information. Let us know if you need any further assistance. Thanks!

I think the issue is with the page names. They don't identify the page they're on, they identify the page you go to next.
Is there a way to identify the page you're on? So I don;t have to vave a cheat sheet that pg 1 = how can we help and pg 2 = shipping

Page 1 = Samples Help Ticket
Page 2 = Shipping Items
Page 3 = Catalogs or Flyers to a Customer
Page 4 = Triage: Catalogs to Sales Reps
Page 5 = Catalogs and Flyers for an Inside Rep
Page 6 = Catalogs and Flyers for an Outside Rep
Page 7 = Samples to a Customer
Page 8 = Triage: Samples to a GB Sales Rep
Page 9 = Samples to GB Inside Rep - NOT Keeping Sample
Page 10 = Samples to GB Inside Rep - Keeping Sample
Page 11 = Samples to GB Outside Sales Rep
Page 12 = Tradeshow
Page 13 = Other
I took the initiative of giving you a list of all your pages so it's easier to follow. The page names are correct, they correspond to the actual page where they're supposed to be.
e.g.
This condition works because Page 13 = Other
If the page names don't match, the condition above shouldn't have worked, along with the rest of the Pages I highlighted in yellow.
To track pages and their names easier, use the nav bar at the bottom to jump to different pages.
