form saying disabled

    I have tried everything and I  can't figure out why my form is saying disabled. Everything is enabled on my end. Please help me!!!!!

    The reason is that you created a form to request critical credit card info on plain text fields.

    Please be aware that this is illegal because it's considered a phishing activity, which violates our terms of use. Card number, expiration date, security codes and card holder name are considered critical information.  The only services allowed to collect  such data are those that are DSS/PCI certified.

     

    In light of this, it is a must to take any of the following options:

    1. Integrate your form with any of our Payment integrations.  

    2. You can take advantage of a feature inside of most of our payment integrations called "Authorization Only" which allows you to collect payments manually at a later time.     I have removed the fields in the form. But you must refrain of making new forms like that, otherwise, we will be obligated to suspend your account again.
