How to remove the Sample Logo form cover?

I have made some modifications to a cloned form. since make these changes, I see a 'Sample Logo' appearing at the top of my published form. I cannot find a way to get rid of this annoying image.

Hello Gary, you can remove that from the ADVANCED DESIGNER, here's how:

1. Click the blue FORM DESIGNER icon on the right

2. Scroll down to the bottom and click ADVANCED DESIGNER

3. Expand the FORM COVER section

4. Click REMOVE

5. Then SAVE
