Once the form is filled out, it does not allow submission if more than one item is selected. NEVER HAD THIS TO HAPPEN BEFORE.

Please check if Unique submissions or Unique questions are not turned on in your form, here is how to do that:
If it is set to "No Check" then it's ok, but if it is set to any other option switch it to "No Check".
