    I have checked the web page where the form is embedded and see that it is not displaying on mobile devices. I see that the following CSS code on your website,

    @media screen and (max-device-width: 1024px)

    .mediumScreenDisabled {

        display: none;

    }

    Please change the display to block from none so that the form should be displaying correctly.

    Hope this information helps!

    I see that the change is already made in the website CSS file and the form is being displayed on the mobile devices normally. Please let us know if you need any further assistance. We will be happy to assist.
