    Can you please tell us what account that was suspended? I've checked your email and it seems that your email (kristybodden16@gmail.com) has no associated JotForm account.

    I checked the account associated with the email (TaliaMorenia@gmail.com) and the account was auto-suspended by the Anti-Phishing Tool. That's why you forms has been disabled. I checked your forms and it seems that there are fields that are against our  Terms of Use. Hence, I have now re-activated your account.
