Can't log in

I was trying to correct my form to remove the password field and it got auto suspended before I could get it fixed, please unlock my account so I can fix it.

I went ahead and deleted the password field that violates our Terms of Use in your form. I also re-activated your account with username primsy as it is suspended. 

However, please note that you seem to have embedded your form directly on your website using its raw source code, meaning that changes on the form using the form builder will NOT reflect on your form embedded on your website. You need to get a fresh copy of the source code and re-embed it if you make changes on the form using the form builder.
