How to send submission data to excel worksheet?

Não estou conseguindo armazenar minhas respostas em uma planilha, como devo fazer isso ??

There are multiple options to send submission data to excel file:

agora  estou cm outro problema ...

Como imprimir na planilha apenas as opções marcadas pelo cliente, para que não fique tão grande a planilha evitando erros na  hora de atender o cliente facilitando visualmente.
