Not sure what you mean by "Prequalify a person". If you need to gather the persons basic information such as name, email, and phone number, you can use the native fields. What is exactly what you are trying to accomplish?

    Hi!
Thanks for getting back to me. I would need to gather credit card information as well as credit score information. So they would need to enter credit card information before submitting the form. Sort of like when you buy a house, how you have to prequalify first. I would need to prequalify an individual for a certain dollar amount, etc. before accepting them as a client. Is this possible? Please advise. And thanks again.
    -Kind regards, Joe

Unfortunately, this will not be possible, because we do not allow asking for credit card information as part of our terms of use: https://www.jotform.com/help/8-Terms-of-Use

"Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."
