    I have had my access denied for my IP address

    Hi,

    Yesterday evening while duplicating and editing one of my existing forms a window appeared stating that my access had been denied.

    The details were:

    Timestamp: Tue, 19 Jun 2012 15:52:02 -0700

    Your IP address: 78.146.64.243

    Requested URL: www.jotform.com/

    Error reference number: 1006

    Server ID: FL_21F1

    Process ID: PID_1340146322.743-1-602909743

     

    I'm not aware that i've broken any rules and if I have I apologise. Would it be possible for someone to look into this. I have found Kotform extremely useful and would like to continue using it.

    My guess is that your account was suspended by someone on our team due to the form named CareSure Download form that has just two fields:


    The person who took the action would have done so due to suspicions about the form being used for phishing purposes. Would you care to explain exactly how the form in question was intended to be used?

    Hi,

    Thanks for the quick reply. My client has asked me to add a resources page to their site and have a .pdf of a printed brochure of theirs available for download. They requested that I added a way of collecting the name and email address of anyone who downloaded it so I thought I would just use an enquiry form I had already generated with Jotform and simply remove the 'enquiry' part. Is this OK?
