Uploading a form already created

I am the director of a preschool in Barrington, Illinois and I am looking for a company to help us post our registration forms online so the families can fill them out digitally and return them to me via email. Some of the forms will also require a digital signature. I am not interested in rebuilding the forms, they have been created to meet all IL DCFS requirements. ps attached below is one form that we would like to use. 

Unfortunately, it is not currently possible to convert PDF forms to web forms.  If you wanted to create web version of those forms, they would need to be created in JotForm from scratch.
