I would suggest the same thing, edit the form and make sure to save the email template. If the issue persists, I would recommend re-creating the whole thing.
1. Delete the notification/autoresponder (https://www.jotform.com/help/132-How-to-Delete-Notifications-Autoresponder).
2. Create a new one:
