    Hi. The application link I sent does not work. Please advise.

    Form that you provided has only one page break field. That is a PDF form that you imported. Most likely PDF form was not imported correctly. You can try to import your form again. It is also possible that PDF form that you have can not be imported properly. 

    Also please note that imported PDF forms can not be edited.

    I would suggest you to create online form using our Form Builder. When you create your form you can then convert it to Fillable PDF form.
