Google Spreadsheet: How do I get data from widgets on separated columns?

    We want to design a form that would create multiple submissions from one form. This form will be used by coaches in the field to evaluate a team of athletes. For efficiency, I want the coach to be able to evaluate all of his athletes on one form, but when he submits the form each athlete would have their own submission (We want each athlete to have their own row of data when exporting to excel/csv). This data would get pulled into a data analytics platform that would allow us to visualize each athletes scoring based on the coaches responses in the field. 

    The linked form (https://form.jotform.com/70606928366969) is as close as we could get to an optimal design, but the corresponding submission/export is not usable for our purposes. It doesn't list all athletes per response, and athletes are grouped into one cell per question (see screenshot). We want a column per question, and a row per athlete, with the responses (does meet or doesn't meet) in the cell. Generated from a form that generally looks like the one we have. Is there a way to change how responses are stored in  the system to meet our needs? If not, do you have any recommendations for how to achieve a form with these qualities?

    I have reviewed your form and I can see you're using the Visual Multiselect widget to list your options, do note that due to the widgets nature you will get the info in that way, for example, each selected value will be separated by commas (,). 

    If you would require to have the records separated as a single field (since each column is a field) you need to use a basic field or use separated widgets to capture each data, this way you could data the data separated as well in your spreadsheet.
