Embedded jot form not displaying on Android phone

I can see the form from the webpage over the internet but not on an android phone.  The webpage displays fine but no form at all on Android phone.  It is this way on both websites.

I have added the next CSS to your form to fix the form height issue on Android phones.

Your site does not have special mobile version, so the user will need to scroll to the right to be able to see the form.
