Not able to create PDF form.

I am having the same problem, however it is march 2017 now - any news?

I just cloned your form: http://www.jotform.us/form/70585866187169, and I was able to create a PDF form from the cloned version in a few seconds. Can you try making a cloned version of your form: https://www.jotform.com/help/27-How-to-clone-an-existing-form-from-your-account 

Then, try creating the PDF version from the clone: https://www.jotform.com/help/212-How-to-Create-a-PDF-Form
