Can a submitter, remove themselves

We have a form for prospective players to fill out when looking for a softball team.  Would there be a way, that a form submitter, can click a link, to:

1) Remove themselves off the list

2) Send an email notification of doing so (we keep tally's that's all)

Submitter wont be able to remove his data but can edit his data. 

The workaround can be that you add an additional field on form  with values Active and Remove Me 

and set up the email notified to send an edit form link.

When the user changes the value in the form you will be notified.
