What is happening with our form?

    Hi - When our form is completed, it tells us that is it incomplete, even though it isn't.

    I can see you're using the source code to embed your form, upon checking the fields names I can see some are different in the embedded form. 

    This may be why you're getting the required fields empty message, what I would suggest you is to get a fresh copy of the source code, please avoid to change the fields names and if possible do not remove their default classes or IDs, this will help to properly recognize the fields and submit them to JotForm, otherwise if a field is not found it will be as empty required field.
