Ive created a form but somehow it is disabled

Ive used your system to job application form like 4-5 times but no matter what I try it always said that my form is nowhere to be found and it is suspended. Im an IT specialist of a clothing company in Turkey and I am not planning to do phishing or whatever you name it. Im simply trying to set up a job application form. Please help me with my problem...
