auto field fill in in a form for 300+ users to save repetitive typing

1. we have over 300 teachers in a data base with all their personal information.

2. what I would like is for them to fill in the first part of their name in the  Last Name field

3. when that happens all the matches would appear in a dropdown. 

4. then they select the correct Last Name and all thel other fields would populate based on their selection. similar to what happens with google auto form fill in.

i.e. When I select my name the address, Pcode, Email, phone number etc fileds are all filled in the appropriate fields

Is this possible?
