Why my form does not work on mobile devices?

I made a form on the computer but I cannot access my form on my tablet.

Do note that JotForm forms are mobile responsive by default, they should work fine in a desktop computer as well as mobile devices. 

I have just tested your form on an Android device and I can see it works fine, here's a screenshot about it: 

Could you please provide us more details about how it shows on your end?
