my client tested these two forms and they do not reflect what i designed and what is showing on their website!

I have checked the email templates for your forms and I can see they're showing fine, please do note that you will have the answers in a table and you will not get the exact layout like the web form.
You could also include the edit link to the form so you can see the web form with the already submitted data.

i don't understand why i have so many click to edit fields if i do not have that in my form set up!
the form is not functional if that is all is capable.

That's because you have not assigned labels to some widgets on your form, you can always edit the email notification to remove that text and leave the answer only. You could also customize the email with your own text.
To do it, go to the settings tab and emails section, click on the edit button to customize the email:

I'm sorry I am not clear on why the email receipt picks up some questions and not others?

The email notification includes all questions as long as you do not manually remove them, when you add a question, it does not have a label by default and it is set to "Click to edit..." once you edit this label it should be also updated in the email notification, somehow the labels in the email notification did not update properly and this is why you can see these text in the email template.
I can see you have already changed your questions labels on your form, for example, in the email template it shows this:
But it belongs to the question:
To avoid to update each single field with this issue, I would suggest you to delete the email notification you currently have, adding a new one will get the updated fields labels and show the correct data.

thanks for your help. i deleted and then added notifications and autoresponders. too bad it doesn't update after form changes!

I have tested and seems the labels are not saving only when you edit the template in the v4 builder, if you edit the form and email template in the v3 builder you will find it works, so I would suggest you to make this change in the v3 builder so you get properly the fields labels updated.
To get the v3 builder you could click on the more button and select the open in v3 option:

Our developers investigated this issue and it turns out that the bug is actually on the side of the old builder.
Let me explain it this way, in the new builder if you go to your emails and change anything on the body of the email templates, the builder will now update the labels when you change label names in the builder in order to keep your email template intact. This was not checked on the old builder and due to that the email templates were constantly losing their design.
Let us know if you ever need anything else,
