Hi, we are looking for a way to email donors who have given to our capital campaign. We want to confirm their recognition name that is going to be listed on a display on the building. I would like to be able to send an email with the recognition name that I have been keeping in our donor database. If they would like to use the name I have in our database, then just click a button that says "Yes, confirm." If they want it to say something else, then a text box that they fill in what they want it to say. The data is kept in a place that I can download it.
Can Jotform do something like that?

If I understood you correctly you want to send your users a form inside of which you will ask them if they would like to use the name or if they want to say something else.
This is possible to achieve with JotForm.

I have set condition to show text box filed only if No option is selected.

I have added a Name field, so that your users can enter their name, so that you are able to compare it with your database.
All submitted data will be stored on our servers and you will be able to view it on submission page of your form.
