Fields are overlapping on ipad only

    Hi. I have a form that is working well on PC and mobile phones but two of the fields are overlapping on the ipad.  Screenshot attached.

    On my end they are overlapping on PC as well. They are overlapping on standalone version of your form as well.

    To resolve this issue set Label Alignment of your email field to Top.

    I would also suggest you to add this CSS code to your form, to increase the width of your phone number fields:

    .mask-phone-number {width:150px;}

    When I inspected your form I noticed another issue. You have fields that collect credit card information and that is against JotForm Terms of use.

    Phishing - You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately.

    Card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.

    I have deleted mentioned fields from your form. Please do not create forms that are violating our Terms of use or your account will be suspended. 

    If you want to have payment fields inside of your form you have to use one of JotForm payment tools.

    If you want to charge your users manually at later date, I would suggest you to use on of payment tools that has built in credit card fields like Paypal Pro, Stripe, Authorize.net, Dwolla.
