I'm trying to place my nre form as a tab in Facebook

Keep getting the following error message. "You must have permission to add apps to a least one Facebook Page to perform the requested action"   Ii this permission on Facebook or Jotform and how do I get it?

The permission requirement to add the facebook tab is on facebook side.
