How can I create a form similar to the one I attached/

I am needing to create a form similar to the one below. We are trying to create an e-form as a convenience to our Contracting Entities and to reduce errors when counting Participants names and the number of meals served.  

It will be very difficult to duplicate the look and feel of the attached form using our Form Builder since it must had been created with a different concept in mind.

The functionality though can be easily applied to our form as well.

I suggest you also check fields and widgets available that emulate matrix, grid and tabular option on a form. Check on Configurable list, Matrix field and grid widgets
