Can you tell me where to find this calculator template on JotForm site?

Hi. We have had this calculator displayed on our website in the past, but I would like to tweak it a little bit. I was not the one who placed the calculator, so I have no idea how to edit it. I got your site name from its HTML code. I can't seem to find a template like this anywhere on your site. Any suggestions?
