Greetings,
We are currently using one of the provided templates to capture project work times for my staff but it is cumbersome to use and doesn't export in a very useful format.
I was hoping you could either suggest another template we should be using, or provide guidance on customizing a template.
Here is our ideal form setup;
Employee Name : ___________________
Date Work Performed:___/___/____
CourseCode Program Work Type Number of Hours
1. AAA-#### XYZ (select from drop-down list item) ##.#
2. AAA-#### LMN (select from drop-down list item) ##.#
3. AAA-#### LMN (select from drop-down list item) ##.#
4. AAA-#### LMN (select from drop-down list item) ##.#
5. AAA-#### LMN (select from drop-down list item) ##.#
6. AAA-#### LMN (select from drop-down list item) ##.#
7. AAA-#### LMN (select from drop-down list item) ##.#
8. AAA-#### LMN (select from drop-down list item) ##.#
9. AAA-#### LMN (select from drop-down list item) ##.#
10. AAA-#### LMN (select from drop-down list item) ##.#SUBMIT
Is this possible?

Thank you for contacting us.
I would like to suggest you a workaround as I understood the content of your form. You would like to add side by side fields for the above fields and all or related ones should be filled. In this case, our configurable list widget seems a perfect fit.
I prepared a demo form for your information. I decided all the fields as Dropdown. This allows user to select it from there. Once a row is finished and if the user need to fill another one, s/he just need to click + to add more field.
Feel free to clone my demo form. If you need any further help by our side, please let us know. We would be pleased to assist you.
