Why are my forms all of a sudden disabled? I'm a premium paid user?

    I just got reports from potential customers that their submissions state 'form disabled'. I was already a premium paid customer but nevertheless I frantically scrambled to pay AGAIN thinking that maybe my premium status has elapsed but NO. That's not it at all. My form is STILL disabled! HELP! I am right in the middle of a campaign and losign submissions!

    I checked your account and the form link posted above. They all work fine. I wonder if the form ID on the page is incorrect. Can you post the URL of the page that has this error?
