Why isn't my slider showing up on my site?

for some reason my slider on one of my applications for a website isn't showing anymore. this is what it should look like (what it looks like in jotform)

But the slider doesn't show on my website, instead it shows like this here

I have checked your form. You already embedded your form into your website with iFrame. It is seen that the changes on your form (in form builder) is reflected to the embedded form directly.

I also couldn't see the second slider in your form.

Could you please check again? Please try to replicate your issue and if your replicate, please provide some screenshots etc.
