Show upload and input fields based on conditional logic

Hello,

I'd like to create a form that accepts artwork submissions (up to a maximum of 6).

Each image to be uploaded must have input fields containing additional information (title, year created, dimensions, etc).

Users are not obliged to upload 6, but 6 is the maximum.

I am thinking that I need to have a conditional statement, but not sure how to do this.

You might consider adding Form Collapse sections for associated Check Box options and show those collapse sections using Show conditions.
