Urgent: Forms are Disabled

All of my forms are disabled and not available.  I was working on a form for an app and I got a Phishing warning.  I was just testing the form.  Now all of my forms are disabled.  I submitted a response to the Phishing error over an hour ago and the forms are still down.  This is a paid account and not acceptable.  Please enable the forms asap.

Hi Jim, I found your account was suspended by us due to a suspicious form titled " Raven Game Stats " which has violated our terms of usage since it is asking for game account nicknames or usernames.

May we know what the purpose of this form is and where is it being used?

Alternatively, you can elect to remove it and we'll go ahead and reactivate your account.
