After creating this form it takes forever for the form to load properly to be filled out OR just does not load at all which results in a "jotform took too long" error.Page URL:

I test your form but I could not reproduce the reported issue at this time.
Can you please try first closing your browser, open again and then test the form again. Perhaps its a temporary glitch only due to browser cache issue.
Let us know if issue persist.
