I found couple forms are disabled

what's the reason?

You have been suspended. One of your forms is created to collect Bank Details which is the reason for the suspension. It's the "eLong Contract" form. This type of information is private and sensitive, we do not allow it as it is against our Terms of Use (https://www.jotform.com/help/8-Terms-of-Use).

I have reactivated your account, but I have deleted the said fields. Please refrain from adding them again. Now, if you want to collect those, considering that your clients will agree to it, I would suggest doing it by either calling them or asking them to submit it through direct email.
