Account Suspended?

Hey there. Apparently my account  got supsended, but I think that wasn't right. I was making a form so that people can get a free picture of their minecraft character, and I need their username, not password or anything private, but just username. I need it so that I can get their skin and make the minecraft character picture with the players skin/look. I am just trying to give out free images, and not cause any harm. 

Please help me...

I have unsuspended the account, the system marked your form with a high % of being suspicious of phsihing activity due to the word "username" which is in the system black list and it automatically suspend forms like these as most of the times, this is used on phishing forms. These forms are built to steal login credentials or credit card information, so we had to implement an automated method to prevent phishing.
