My client has made me aware that they have not received an online appointment form off the website - it was an appointment for a vet practice for their dog. I was able to see the form in my jotform account.
Can you please check for me that there isn't something wrong on the jotform end as it is important that they receive these forms.

You form Autoresponder configuration looks good. Please check if user's email is listed in our bounce list, here is the guide: How to Remove Your Email Address from Bounce List
The email address is usually added to our bounce list if we receive a rejection message from recipient mail server.

Many thanks for such a speedy reply. I have checked and the e-mail address is not in the bounce list - is there anything else that could be causing them not to receive a message? Thank you for all your help with this.

If the issue continues, our best recommendation is to use your own SMTP settings for the sender email: https://www.jotform.com/help/244-How-to-Setup-SMTP-for-a-Form
