Hi jotform team,
I have a form that has been in use for a few months and today all of a sudden a CAPTCHA field appears after you hit the submit button on the form. How do I prevent this from happening and why did it start happening out of nowhere without any changes on my end?

I was able to replicate mentioned issue when I tested your form and when I tested my cloned form.
It seems that there is an issue with some widgets that are added inside of Version 3 of our Form Builder. In your case Mobile Responsive widget is causing this issue. It breaks html layout, so form is not passing the check and asking for captcha.
I will escalate this issue to our developers and we will inform you via this thread once this issue is resolved.
For now I can only suggest you to remove Mobile Responsive widget form your form, until this issue is resolved.

Hey Azhar,
This issue is fixed now. I also checked your other forms and all of them are fine.
Let us know if you ever need anything else,
Cheers.
