How can I fill, issue, print a form?

I need a form that I can fill in and then take a printout. The printout will be the physical receipt that I will handover to the other person who pays me money. So I also need a sequentially increasing receipt no. to be printed.

From time to time, I will download the data and pass it on to our accounts department for making entries of moneys received on a daily or periodical basis.

This also means that the form will always be filled in and printed by us rather than the person payment the money.

1. Can I do it?

2. If so, can you help through the steps?

3. How much will it cost for, say, 10,000 such receipts or forms per month?
