Moving a form into my account

Hello,

A friend started  a form for me. She made me a collaborator so that I could edit the form and finish developing it. I created my own account, copied the form and completed it. It is still not my form. 

How do I get the form out of her account so I can use and edit it without her having access? This is a form that will have sensitive information input into it.

The form belongs to the account AKAHartfordTechnology so we need to receive the request from this account, please ask the form's owner to open a thread and ask to transfer the form to your account, provide us the username  of the target account as well.
