my forms are missing and when I try and log into my account it says auto suspended. What does this mean and how do I fix it?

Your account status is autosuspended. Your form is asking sensitive Credit Card Information. This is a violation of our terms of use. I will re-activate your account but you need to immediately delete the field that is asking for Credit Card Information or you can delete the whole form. Your account might be suspended again if you don't do this immediately.

to solve a problem we were having with our website store coding. I never
used the form but forgot to delete it. I appreciate your help. I will find
the form and delete it right away!

my account now but my forms are saying they are disabled and I can't find
where to enable them. I deleted the form right away as you said. but for
some reason this is happening.

Hello,
I checked your account and I noticed it is auto-suspended by our anti-phishing filter, I have also checked your forms and I found them both are built in accordance with our Terms of Use. In sight of that, I reactivated the account as suspension was due to an error.
