    Hello Mellanie

Unfortunately , that is not possible. Our system is only able to import forms that  begin with a <form> tag and has form elements that begin with the <input> tag.

    Best regards

However, since the form you are looking for is very easy to make , you can go ahead and open a free account, then clone this form  I have prepared for you

You can check these guides in order to have more details about how to create and improve forms

    http://www.jotform.com/help/2-How-to-Create-Your-First-Web-Form

This guide doesn't include email setup or embeding forms , so please check them as well

Should you need further assistance, let me know
