I was able to replicate mentioned issue on my cloned form as well. This issue is related to Preview Before Submit widget. To resolve your issue update your widget inside of Form Builder.
On my cloned form that has resolved the issue.

Well, I updated the widget, as per your suggestion. All seems to be well now.
But why do these kinds of things have to happen?

There was a temporary issues with some widgets added via v3 editor yesterday. It was fixed by our developers within a day.
