    How to set up a drop down that creates different text boxes underneath?

    I would like to create a drop-down which lists who the form submission is coming from- an individual, a collective, a company, or a group of individuals.

    The fields underneath would change according to the drop down menu. So if the submitter chose individual, the fields underneath would change to have the First Name and Last Name field. But if they chose Company, the field would change to Company Name. If they choose group, multiple name fields would open up underneath.

    Is this possible?
