How do I get multiple selection boxes to function

G'day folks I created a form called request a quotation. On that form I put check boxes. I would like people entering the website to be able to select one, two or many services, but the form is currently only allowing for one box to be check. Im certain its not the form, but me not having enough knowledge of how to do this. If some one can let me know how to fix this I will be very greatful.

Instead of using a Single Choice Field that allows only one option, please use the Multiple Choice Field which will allow the user to select more than one choices.
