Why have all my forms been disabled?

    Your account was autosuspended because our system's filter found a form which can be suspected to be malicious. Upon reviewing your forms, I found one which collects a username and password. Please take note that we do not allow collection of such information and is a violation of our terms of use.

    On this matter, we'll be able to reactivate your account only if you agree to remove the fields for password collection.

    I agree to remove those fields today on my form.

    Hi Rachel, I believe you meant to send your reply to another ticket? or do you own the account "maddogzs" too?

    In any case, I have removed the offending field and re-activated the account maddogzs because all the other forms are in compliance with our terms of use.
