Where are my headers in the PDF?

Hi - need a bit of help with the form submission process please.

Designed a form with headers, which displays fine on the completion part of the process, but this form needs to retain its headers when converted to pdf. How can I do this?

Please try to enable the "Show Headers and Texts" option in the Submission page. Here's how:

1. In My Forms page, select the form and then click the "Submissions" button.

2. Click the "Options" icon and then enable the "Show Headers and Texts". After that, click the "Options" icon again.
