Fields Disappeared from form

As of current.  All the fields except for name and address have disappeared from both of the forms on our account.  https://form.jotform.com/70227816932155 .   Also.  The view-able submissions have also deleted all but the name and address.  Please assist us in resolving this issue.  It has not been modified since made (January 24, 2017)

You should be able to restore your form to the correct state using the revision history:
