I just created my first form and when I try to preview it, it says it cannot be found and is disabled?

I tried clicking on enable in the More pull down menu on the main page, but only disable was an option.  So I disabled it and then re enabled it, but still no go.  Could you please tell me what I'm doing wrong?

This happens when you're asking for info that goes against our terms; however, I've reviewed your form and I can see you're not asking for such info so I have reactivated your account.
